Passport application appointments are offered at our branch libraries
PLEASE NOTE: Passport appointments are released on a next-business-day basis. Appointment times are released after 5 pm. Due to high demand for passport services, appointments are typically filled up within 20 minutes of them being released.
Before scheduling an appointment please visit the U.S. Department of State website for the most up-to-date information on passport requirements and renewals.
IMPORTANT:
YOU ARE REQUIRED TO BRING THESE ITEMS TO YOUR APPOINTMENT:
1. Completed application (DS11)
- Application must be signed in front of the passport acceptance agent.
- Application must be filled out in BLACK INK.
- No correction fluid or correction tape such as Wite-Out and similar brands.
2. Evidence of U.S. Citizenship: Bring Original and Two Black & White Photocopies. (All documents must be original)
- Previous U.S. passport*
- Original or certified copy of birth certificate, OR
- Consular Report of Birth Abroad or Certification of Birth OR
- Naturalization Certificate, OR
- Certificate of Citizenship
*Note: For Minors under Age 16 you must establish proof of citizenship AND proof of relationship. Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s). See the State Department's website for further information for minors under 16.
3. Identification: Bring Original and One Photocopy of Front & Back
- Submit a photocopy showing the front and back of the original identification document being presented. Copy must be one-sided on plain white, 8 1/2" X 11" standard paper.
- Identification must be one of the following:
- Current, valid driver’s license
- Government ID: city, state or federal
- Military ID: military and dependents
4. Passport Photos
- Photo Service is available at the Library for $15 (check or money order ONLY), OR
- Bring 1 color passport photo, 2x2 inches in size, taken within the past 6 months (for more information on acceptable passport photos visit the U.S. Department of State Passport Photos website).
- Photos for children ages 4 and under are not available at our facilities.
5. TWO (2) Required Payments
- BOTH payments must be separate and MUST be personal checks, money orders, or cashiers checks. NO CASH, CREDIT, OR DEBIT CARDS.
- One personal check, money order, or cashiers check payable to the U.S. Department of State, AND
- One personal check, money order, or cashiers check payable to the City of Chula Vista
- One personal check, money order, or cashiers check payable to the U.S. Department of State, AND

- BOTH parents must appear and present evidence that they are the child’s parents or legal guardians for a minor under 16 years of age.
- ALL applicants must be present.
- Please allow approximately 6 - 8 weeks for routine application processing. Go to travel.state.gov for the most up-to-date processing times.
- Track the status of your application here (allow 2 weeks after submitting your application before checking status).
- Passport Card is valid for land and sea crossings between the U.S. and Mexico, Canada, Bermuda and the Caribbean only.
