If card room permit is EXPIRED MORE THAN TWO WEEKS, employee MUST complete and submit a new Police Controlled License application and a $175.00 processing fee.

ALL FEES ARE NON-REFUNDABLE.

YOU MAY NOT OPERATE IN THE CITY OF CHULA VISTA WITHOUT A VALID POLICE CONTROLLED LICENSE/PERMIT.

You will be notified by your employer or by email when your ID badge is ready for pick up.

Required Documents/Fees

  • Card Room Employee Renewal application
  • Check, Cash, Credit Card or Money Order:
    • $100.00 renewal fee payable to the City of Chula Vista
  • Submit (1) 2” x 2” photo taken within the last six months.
