The following information must be submitted with your application prior to the approval of your permit. Applications will be accepted at the Chula Vista Police Department on a walk-in basis on Tuesdays from 9:00 – 11:30 AM or 1:00 – 4:00 PM or by appointment by calling 619-691-5244.

Please be aware that an incomplete application submitted by mail will cause delays in the processing of your permit.

Required Documents/Fees

• Completed Police Controlled License
• Signed Statement of Understanding
• Copy of current City of Chula Vista Business License 
• Payment of $322.00 (Checks and money orders are made payable to the City of Chula Vista)

All fees are non-refundable.

Your Police Controlled License is valid for one year from the date of issue.
